Role Description
This is a contract hybrid role for a Case Management Nurse at Assisted Living Locators North Tampa. The compassionate Case Management Nurse will be responsible for tasks such as developing relationships with hospitals, rehabs, skilled nursing and doctors offices. They will also assist with elder care case management. This role is located in Tampa, FL, with the flexibility of remote work. Part time available with the ability to transition to full time.
Qualifications
- Strong background in Nursing and Medicine
- Experience in senior care
- Patient, kind and compassionate
- Experience in Case Management and Utilization Management
- Knowledge of Discharge Planning processes
- Excellent communication and interpersonal skills
- Ability to work independently and collaboratively
- Attention to detail and organizational skills
- Active RN license in the state of Florida
- Certification in Case Management is a plus
