The Impact You Can Make Responsible for managing the delivery of nutrition and diabetes education and support to both inpatients, outpatients and the community, based on approved standards of care as indicated by Federal, State, accreditation standards and practices of the Academy of Nutrition and Dietetics and the (NSDSME) standards.

How You Will Fulfill Your Potential Responsibilities - Develop appropriate interdisciplinary care plans based on current treatment knowledge and clinical practices, discussing care plan with patient and/or significant other and the appropriate staff (i.e. physician and nurse etc.).
- Function as an advocate for patients when implementing care plan.
- Comprehensive nutritional assessment of patients prioritized to be at risk during the screening process or as a requested consult. This is to include both acute care patient and outpatients. Completed assessments which evaluates patients nutritional profile will be documented in a medical record using current technology and resources.
- Respond to requests for nutrition education or counseling for patients and/or groups including appropriate materials for continuation of medical nutrition therapy or wellness.
- Provide in-services on medical nutrition therapy to staff as appropriate.
Education/Accredited Programs - Requires a Bachelors Degree in nutrition and dietetics or a related field.
- Preferred Masters Degree in nutrition and dietetics or a related field.
- Five or more years of experience in clinical practice of dietetics and diabetes management.
- Academic and/or experiential preparation program management.
- Academic and/or experiential preparation in the care of people with a chronic disease.
Licenses/Certifications/Registrations - Registered Dietitian (RD)
- New York Certified Dietitian
Skills/Abilities - Proven management skills, which include a knowledge and application of continuous improvement techniques.
- Excellent communication skills with staff and physicians.
- Computer literate and the ability to learn department specific software.
- Maintains a clear understanding of regulatory requirements.
- Program management experience.

Salary Range The expected base rate for this Glens Falls, New York, United States-based position is $62,940.80 to $94,432 per year . Exact rate is determined on a case-by-case basis commensurate with experience level, as well as education and certifications pertaining to each position which may be above the listed job requirements.
