We have immediate opportunity for a Nurse Practitioner (new grads welcome) to join our Population Health Operations team inthe Inland Empire area. The clinicians we seek are those who practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.
About the Role:
- Serve the Southwestern Valley Complete senior wellness visits in clinics, virtual, and SNF/ custodial settings to see 10-14 patients per day. May be asked to do home visits on occasion.
- Travel to Optum outpatient clinics and independently contracted physician offices, as well as some home and SNF custodial visits, 1-2 locations per day
- Conduct comprehensive assessment, perform and discuss recommended screenings (such as QuantaFlo, Neurometrix, or point-of-care A1C), comprehensively document and code encounter, and conduct patient education and appropriate follow up.
- Work a schedule of Monday – Friday, with occasional need for Saturday clinics.
Required Qualifications:
- Bilingual in Spanish
- Master’s degree from a four-year college and/or a professional certification beyond a four-year college
- Certificate of completion from an accredited Physician Assistant or Nurse Practitioner program
- Active Board Certification through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA)
- Unrestricted Nurse Practitioner or Physician Assistant license in California or ability to obtain before start
- Current CA DEA certificate or ability to obtain prior to start date

For California Residents Only: The salary range for California residents is $135,000-$176,000 per year. Salary Range is defined as total cash compensation at target. The actual range and pay mix of base and bonus is variable based upon experience and metric achievement.
