Pride Health is hiring a Nurse Practitioner for our client in the Virginia Beach area! This is a 3 to 4-month-long contracted position.
The Senior Community Care (SCC) program is a longitudinal, integrated care delivery program that coordinates the delivery and provision of clinical care to members in a facility setting. The clinician within SCC provides care to our highest-risk health plan members and is part of an interdisciplinary team. In this role, you will have the ability to achieve work-life balance. Flexible scheduling is offered where providers can flex their time with no on-call, weekends, or holidays required.
*Required to travel between 3 facilities in the Virginia Beach and Norfolk areas*
Responsibilities:
- Conduct comprehensive assessments
- Effectively manage acute and chronic medical and behavioral conditions
- Ensure accurate and complete documentation, including ICD 10 conditions
- Communicate and collaborate with the interdisciplinary care team
- Conduct advanced illness and advanced care planning conversations
- Provide patients and caregivers with counseling and education
Qualifications:
- Active and unrestricted license in the state in which you reside, or ability to obtain by the start date
- NP: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program, and board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in one of the following: Family Nurse Practitioner, Adult Nurse Practitioner, Gerontology Nurse Practitioner, or Adult-Gerontology Acute Care Nurse Practitioner.
- Current, active DEA licensure/prescriptive authority.
- Ability to complete the job's physical requirements, including lifting a 30-pound bag in and out of the car, navigating stairs and various dwelling conditions, and sitting, standing, and kneeling to perform a physical assessment.
- Fully vaccinated against COVID-19
- 1 year of Long-Term Care experience preferred.
- Understanding of Geriatrics and Chronic Illness.
- Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs.
