This position will support the organization's commitment to quality by performing chart review, supporting the peer review process and providing guidance for LHN process improvement and compliance activity. Works collaboratively with physicians, staff and other health care professionals to provide care coordination across the health care continuum for an identified high risk population within the hospital setting. Is an integral member of the health care team who works to ensure safety, best practice and high quality standards of care are maintained across the continuum. Responsible for coordinating a range of self-management support and disease registry activities for the identified population. Uses data to help manage the health of the identified population. Works closely with the physicians, administration and staff at all facilities to review charts, advise on quality incidents and present data and support quality of care. A critical attribute required of this position is exceptional interpersonal customer services consisting of an outgoing positive attitude, warm friendly joyful demeanor, and the utmost care and respect for our guests, patients, families, customers and colleagues.
EDUCATION
- Education: Registered Nurse required; BSN preferred
- Experience: Minimum three (3) years experience in a hospital,
- Required Licensure: Must have a current Washington State RN license
- Other: Bilingual and/or English-Spanish speaking abilities preferred. Ability to work with a culturally diverse population

Salary Range: $39.93 - $54.03
