DescriptionSummary:
A healthcare professional that assesses, plans, implements, coordinates, monitors and evaluates patient care activities under physician supervision. He/she has clinical and functional knowledge of the position for which they are accountable. He/She collaborates with other associates and departments effectively to promote high quality and effective care.
Responsibilities:
• Responsible for effective management of patient care delivery by utilizing the nursing process. Completes and documents initial patient history, physical and chart documents within 24 hours of initial appointment.
• Completes thorough assessment of patient pre and post wound care treatment according to department policy and procedures.
• Initiates, updates documents, and communicates an appropriate multidisciplinary plan of care based on patient needs; to include medical, psychosocial and spiritual.
• Evaluates patient response to care, revises plan of care as necessary and documents appropriately.
• Actively participates in obtaining all required documentation, improving processes, etc. in order to assure full reimbursement for services. Administers and documents medications/treatments safely and effectively. Assists physician with treatments and procedures.
• Recognizes change or urgent situations in patient condition and intervenes appropriately.
• Patient/Family Education: Provides patient and/or family with information relative to treatment plan, diagnostic/surgical procedures, disease process, and care between visits and after discharge. Obtains required signed consent forms. Documents appropriate teaching, considering age, cultural and religious beliefs, and barriers to learning. Evaluates effectiveness of teaching and provides appropriate follow-up. Strong emphasis on nutrition.
• Provides leadership and direction in accordance with departmental goals and objectives. Provides oversight for LPN, tech, and other associates as indicated.
• Completes all tasks and documentation accurately within set time frame related to AED check, autoclave, glucometer, HBO chamber list, etc.
• Maintains treatment rooms as per policy. Keeps supplies stocked and organized and re-orders as needed.
• Responsible for patients and physician correspondence to include physician appointments, scheduling and rescheduling of patients, referrals, and scheduling of tests and procedures, etc.
Requirements:
Education/Skills
- Associate's Degree in Nursing
Experience
- 1 year of experience in the related nursing specialty preferred
Licenses, Registrations, or Certifications
- BLS required.
- RN License in state of employment or compact.
