Job Title: RN Case Manager Job Specialty: Case Management Job Duration: 13 weeks Shift: Day - 5x8 - 8:00AM - 4:30PM Guaranteed Hours: Experience: Minimum 2 years License: Registered Nurse (RN) Certifications: Basic Life Support (BLS) Must-Have: Proven experience in case management and strong interdisciplinary collaboration skills Job Description: - Coordinate the overall interdisciplinary plan of care for patients, from admission to discharge. - Create a plan for care across the continuum, integrating patient and family preferences and values. - Monitor patient care through assessments, evaluations, and patient records. - Advocate for necessary resources and the removal of barriers to care. - Maintain ongoing dialogue with supervisors and other healthcare providers to ensure effective implementation of the health plan. - Act as a resource for adequate medical record documentation, appropriateness of services as they relate to diagnoses, and treatment options for post-discharge care.
