Position Summary
The RN Patient Navigator is a Professional, with a specific body of knowledge regarding the diagnosis and treatment of specific disease processes. This Nurse primary responsibility is to navigate Patients through the Healthcare system at THOP and to facilitate timely diagnosis to treatment through-put. The RN Patient Navigator demonstrates knowledge of specific diagnoses and facilitates patient and family educational and emotional needs, and the coordination of services. The RN Patient Navigator collaborates with the Medical Staff to provide individualized care to their patient. This individual is also responsible for collecting outcome data related to the program and evaluating the effectiveness of the program.
Position Qualifications
Minimum Education
- Graduate from accredited health care related program (i.e. SW, RN, Health Care Educator etc.)
Required Licenses/Certifications
- Current RN License
- Current BLS for health care providers
Required Skills, Knowledge, and Abilities
- 2 years nursing experience
- Patient Navigation Experience preferred
- Computer skills
