Position: Certified Diabetes Care and Education Specialist (CDCES)

Rate of Pay: $70,000 - $85,000/yr
Exempt/Non-Exempt : Exempt

The Certified Diabetes Care and Education Specialist (CDCES) is a skilled and professional with competence in diabetes and lifestyle medicine. This role works cooperatively and communicates effectively within a multidisciplinary framework to actively promote holistic care for patients and families. The CDCES position demonstrates self-direction, excellent problem-solving ability, and initiative in the role of educator and care and support specialist. The Certified Diabetes Care and Education Specialist (CDCES) is responsible for providing individualized diabetes self-management education/training along with medical nutrition therapy integrated with the patient's medical goals to individuals and groups according to the Scope of Practice, Standards of Practice, and Standards of Professional performance for Diabetes Educators (AADE, 2011).
CDCES duties and Responsibilities include the following.
- Collects DSME program participant assessment data in a collaborative and ongoing manner.
- Collaboratively develops educational and care management goals, learning objectives and a plan for educational content and teaching methods with DSME program participants.
- Provides educational and care management interventions that utilize primarily interactive, collaborative, skill-based training methods and maximizes the use of interactive training methods.
- Collaboratively develops an individualized follow-up plan with each program participant. *
- Evaluates effectiveness of care management services provided by measuring attainment of learning objectives.
- Conducts a follow-up assessment upon completion of DSME program services, using outcome measures from the AADE7.
- Documents assessment data, educational plan, care services provided and evaluation results in each participant’s educational records.
- Utilizes a team approach to provide services and collaborates and communicates with team members when needed.
- Identifies when a program participant’s needs are outside the scope of the instructor’s practice and expertise, plus arranges for additional services to meet needs.
- Participates in the development of training materials.
- Contributes to, and participates in, a continuous quality improvement process to measure DSME program and to identify and address opportunities for improvement.
- Participates in planning and developing diabetes management related policies, procedures, and goals.
Education/Experience:
- Certified Diabetes Care and Education Specialist (CDCES) with at least three years of experience in diabetes care, education, and knowledge in program management.
- Working experience with insulin pumps and continuous glucose monitoring technology preferred.
- Knowledge and experience in the workings of health insurance coverage and system processes preferred, or a desire and willingness to learn and practice new learned skills to support successful patient care.
- Prior experience in DSMES Program management with ADA ERP or ADCES Education Program preferred.
Certificates and Licenses:
- Certified Diabetes Care and Education Specialist (CDCES) in good and current standing with the Certification Board for Diabetes Care & Education (CBDCE)
