This Licensed Practical Nurse (LPN) will be responsible for the following duties:
- Responsible for Start of Care & implementing client care plan
- Consulting and educating clients and family regarding care/services provided by agency
- Performs chart audits to ensure that all documentation is accurate and maintained per federal, state and local program regulations and standards
REQUIREMENTS:
- Graduate of an accredited school of nursing with an Associate Degree in Nursing with one to three years of experience.
- Must demonstrate the ability to work with a wide range of individuals professionally within the Home Health community and or local agencies
- Must have a valid Ohio driver s license and properly insured transportation
