Board Certified Ophthalmologist
Michigan license
BLS/ACLS preferred, only required if performing moderate or deep sedation
Practice Setting: Hospital based
Average number of patients per day - Hospital: 1-2 per month
Other Ophthalmologists provide coverage during Locum's coverage dates: Yes
100% Adult/Geriatric
What Percentage of the Practice is Non-Surgical, Surgical, Laser: 50%
