Lakeside HR Group has been engaged to recruit for an Assisted Living Clinical Nurse Supervisor at one of Minnesota's leading nonprofit senior housing and service providers. The Clinical Supervisor has 24-hour responsibility for administration of day-to-day services for home care clients and Assisted Living residents; responds to tenant and resident emergencies and medical concerns. Operates as the Clinical Nurse Supervisor for Home Services and Assisted Living. Provides ongoing customer service to families and staff. Guides staff in compliance with Home Care and Assisted Living regulations. Has overall responsibility for the clinical management of the tenants/residents and the human resource management of the staff. Develops and administers all aspects of nursing services and other health care services provided by the campus and has direct supervision of Licensed Nurses and Home Health Aids.
Qualifications:
Education: Successful completion of an accredited Nursing program. Current Minnesota licensure as a registered nurse.
Experience:
- 3+ years nursing experience
- 2+ years of home health care experience including Assisted Living and Comprehensive Home Care requirements
- 3+ years supervisory experience

- Demonstrated working knowledge of Federal and State Home Health / Assisted Living rules and regulations; demonstrated ability to accurately interpret and communicate these rules and regulations

2. Coordinates care for tenants and residents requesting home health or assisted living services.

D. Initiates and ensures each client/resident has a current Service Plan and provider orders.

H. Monitors and ensures accurate service billing.
I. Maintains liaison with attending physician and other practitioners.
J. Notifies family in case of client/resident death.
K. Demonstrates knowledge of current health practices when performing observation and evaluations of client/resident care.
3. Assures home care program and Assisted Living is in compliance with applicable law and standards of practice.

4. Conducts the wellness clinic and supervises.

A. Participates in care center/hospital conferences to assess needs for client/resident return.
B. Provides continuity between hospital, care center, and housing.

B. Facilitates nursing quality assurance meetings and develops and implements plans for assuring quality client/resident care.
