Job Opening: Medical Care Coordinator
Location: San Diego, CA 92110
Schedule: Monday-Friday 8:00 am-4:30 pm
The Medical Care Coordinator plays a crucial role in the healthcare system by facilitating communication and coordination between healthcare providers, clients, and in collaboration with Clinical Care Director. This role involves managing the administrative aspects of client care, ensuring a smooth and efficient healthcare experience for assigned individuals. The Medical Care Coordinator is responsible for organizing, implementing, and monitoring healthcare plans to optimize patient outcomes under the direct supervision of the Clinical Care Director
Key Responsibilities:
- Serve as one of the primary points of contact for clients, addressing inquiries, providing information, and offering support throughout the healthcare process.
- Observe clients’ health status and take actions to address concerns within scope of practice.
- Facilitate effective communication between supervisor, clients, healthcare providers and other relevant parties.
- Monitor associates and Supervision with self-administration of prescribed mediation, including frequent audits of medication records.
2. Client Advocacy:
- Advocate for clients to ensure they have access to appropriate shelter and support services.
- Work with community resources to address immediate needs such as food, clothing, and medical care as directed.
- Respond to crises and emergencies, providing immediate support and connecting clients with appropriate resources as directed.
3. Care Plan Development:
- Evaluate resident care needs and determine required level of care upon admission and changes in condition as directed and in coordination with Clinical Care Director.
- Collaborate with healthcare professionals to create and update individualized care plans based on client needs, medical history, and treatment goals.
- Ensure that care plans are comprehensive, realistic, and aligned with the patient's overall health objectives.
4. Appointment Scheduling:
- Assist with and Coordination and scheduling medical appointments, tests, and procedures for patients in collaboration with healthcare providers and medical facilities.
5. Healthcare Resource Coordination:
- Assist clients in accessing necessary healthcare resources, including medications, medical equipment, and support services.
- Collaborate with external agencies, healthcare providers, pharmacies, insurance providers and community organizations to streamline access to additional services and healthcare resources as directed.
6. Documentation and Record Keeping:
- Maintain accurate and up-to-date records of patient information, care plans, and communication with healthcare providers.
- Upload vital documentation into the electronic medical records as appropriate.
- Ensure compliance with healthcare regulations and privacy standards in all documentation.
7. Insurance Coordination:
- Work with insurance companies to verify coverage, obtain authorizations, and address any billing or claims issues under the direction and supervision of the Clinical Care Director
- Educate clients on their insurance coverage and assist in resolving insurance related concerns as appropriate.
8. Patient Education:
- Provide patients with information about their medical conditions, treatment options, and self-care strategies under the direction and direct supervision of the facility Clinical Care Director
9. Collaboration with Healthcare Team:
- Work closely with case managers, social workers, and other internal departments to ensure a coordinated and supportive environment for clients under instruction from facility Clinical Care Director.
- Foster strong communication and collaboration with physicians, nurses, social workers, and other members of the healthcare team.
- Participate in regular team meetings to discuss patient progress and address any challenges in care coordination under the direction of the facility Clinical Care Director.
- Assist in the development, coordination and orientation of new department personnel.
10. Community Outreach:
- Engage in community outreach efforts to raise awareness of shelter services and resources as appropriate.
- Foster collaborative relationships with local organizations, businesses, and government agencies to strengthen support networks as part of the community team in cooperation with facility wide events and as appropriate.
Qualifications:
- Supervisory experience in a clinical care role.
- 5+ years of prior experience in the medical field as an MA OR 3+ years as an CNA or LVN.
- Experience working with SNF or MD offices preferred.
- Previous experience in healthcare coordination, case management, or a related role.
- Strong communication, organizational, and interpersonal skills.
- Knowledge of medical terminology, healthcare regulations, and insurance processes highly desired.
- Ability to work independently and as part of a multidisciplinary healthcare team.
Compensation: $65,000- $80,000 plus benefits
