Job SummaryThe LPN Care Coordinator is responsible for the maintenance of clinicians’ weekend schedules. This role also coordinates patient needs by processing and relaying information to appropriate resources and obtaining essential information from outside resources. Assists the team with the monitoring of the telehealth patients. Manages referrals for home care services and insurance authorizations. Works closely with Clinical Supervisors and the Intake Department to coordinate coverage of admissions and evaluations, while continuing to meet current patient needs.
Core Job Responsibilities- Manages and assists with telehealth services and home monitoring system requiring patient interaction via phone.
- Enters patient information, referral and insurance information into the EMR (Electronic Medical Record) per call center processes. Referrals may include Home Health Care Services for nursing, physical, occupational and speech therapy, home health aides, social work, and respiratory therapy.
- Receives referrals and coordinates the day-today scheduling of resources/visits to meet physician orders.
- Provides technical support and acts as a resource to the Clinical Manager and team members.
- Functions as a resource to internal and external referral services and providers in the community.
- Refers or directs callers to appropriate resources bump down
- Schedules visits per established processes. Ensures clinician schedules have all pertinent visit and patient details.
- Communicates patient visit schedules and details with clinicians. Shares schedule updates or changes promptly.
- Interacts with clinicians regarding patient schedules and updates or changes.
- Ensures flow of patient information and documentation is complete, accurate and directed to appropriate area promptly.
- Views and investigates all “open” visits in EMR to ensure coverage.
- Assists in covering clinician schedules due to sick calls and other unexpected changes with staff. Also provides back-up coverage to other schedulers within the department.
- Coordinates patient supply needs and documents in the EMR.
- Serves as a patient advocate when needed.
- Prepares reports as needed.
- Participates in the orientation and mentoring of new staff and promotes collaborative teamwork.
- Adheres to applicable federal/state regulations and all MVHS policies.
- Is flexible to cover holidays and weekends on a rotating schedule with other staff.
- Perform other duties as required.
Education/Experience RequirementsREQUIRED:
- Minimum of two years’ experience working as a practical nurse, preferably in an acute care setting.
- Ability to effectively communicate with patients/families/referral sources & Insurances.
- Strong working knowledge of Home Care Services (VNA/Senior Network Health (SNH)) and community resources (i.e.; MOW, Life Line, etc.).
- Strong organizational and time management skills.
- Computer literacy with proficiency in the Microsoft Office Suite, including but not limited to data entry, retrieval, and report generation.
Licensure/Certification RequirementsREQUIRED:
- Licensed and currently registered to practice as a LPN in New York State
