Full-Time Position:
- Salaried M-F: 8am – 5pm
- Rotating weekend, holiday, and after hours on-call phone support

Summary:
Under the direction of the Medical Director, the Physician provides medical care and education to patients and families in a variety of settings, inpatient, outpatient, home, and facility based, for both non-hospice palliative care (Keystone Medical Services) and hospice patients (Hospice of Holland).
Responsibilities of the Physician:
- Provides medical consultation to all Hospice personnel of the various programs in Hospice of Holland. Assumes the responsibility of primary physician for patients admitted to Hospice without a qualified primary care physician.
- Participates in admission and re-certification process and in the weekly interdisciplinary team meetings.
- Makes visits to patients as appropriate i.e. acuity, date of admission, pain and symptom management, meet regulatory requirements, etc.
- Provides consultation as needed with any patient's attending physician regarding symptom control and/or other clinical care plan issues. Serves as a consultant to community physicians regarding potential Hospice patients.
- Assists in the Palliative Care program (Keystone Medical Services), collaborating on standing protocols, practice standards and outcome measurements. Sees palliative patients on an inpatient or outpatient basis.
- Reviews patient's medical diagnosis, prognosis, medications, procedures and clinical course, as appropriate to ensure cost-effectiveness and Medicare compliance.
- Develops and maintains effective and positive relationships with health care professionals within the community. Serves as a source of end-of-life medical expertise, educating and promoting the benefits of hospice care.
- Participates with the Medical Director in establishing a call schedule to provide twenty-four (24) hour, seven (7) days a week coverage of physician services.
- Understands and assures compliance of regulations and standards particular to state, federal, and accreditation associated programs pertinent to patient care.

Physician Qualifications:
- Possesses current license, (M.D. or D.O.) to practice medicine in the state of Michigan.
- Has board eligibility/certification in internal medicine, family medicine, oncology, emergency medicine, or hospice and palliative medicine. Active board certification is preferred.
- Is board certified in palliative medicine, or will become board certified in palliative medicine, within 2 years of hire. Fellowship training is preferred.
- Has practiced medicine for at least three years.
