This position is accountable for developing and implementing a community-based housing navigation and social determinants of health program, designed to connect low income or Medicaid beneficiaries with appropriate affordable housing units, resources and support services to successfully maintain residency.
This is a home-based office position with field responsibilities up to 50% of the time, pending on business needs.
If you are located in Southeast Region of Michigan, you will have the flexibility to telecommute as you take on some tough challenges. Per the business needs this position may require travelling outside of the Southeast region.
Primary Responsibilities:
- Conduct screening interviews, complete intake documentation and coordinate the location of appropriate housing units and / or supportive services
- This work includes implementing processes to manage the intake and referral of individuals experiencing a housing issue
- Collaborating with national UnitedHealthcare teams to develop a community based or regionalized housing coordination strategy
- Assist in the management of a program to better connect members and low income individuals with appropriate housing and housing support services including developing a plan of intervention that addresses the needs for affordable housing including housing assessments and crisis intervention and continuity of care coordination
- Initiation and development of relationships with housing-focused community organizations, landlords, affordable housing developers and other housing service providers to identify new and existing opportunities for accessing resources
- Engagement with individual member cases to identify appropriate housing (such as subsidized housing, Section 8, affordable and market rate housing) or housing support services (such as rental assistance, financial counseling, foreclosure mitigation, etc.) to place or maintain affordable / appropriate housing
- This includes the identification of need and dissemination of referral materials
- Collaboration with state agencies, community organizations and other social service agencies to promote and grow housing opportunities within the region and / or state
- Coordinate with other social determinant of health coordinators (e.g. employment, transportation, financial services, etc.) to facilitate access to support services to sponsor successful tenancy and whole person coordination

Required Qualifications: - Bachelor's Degree in Business, Health Administration, Public Policy, Social Work, or a related field OR Associate Degree with 2+ years of experience in housing agencies, housing policy, local housing organizations, public housing authorities, housing subsidies/vouchers, and other low-income housing support programs
- 2+ years of experience with housing agencies, housing policy, local housing organizations, public housing authorities, housing subsidies/vouchers, and other low-income housing support programs, specifically in Michigan
- Intermediate level of experience with project/program management, including action planning, timelines, and business requirement definition
- Intermediate level of proficiency in PowerPoint
- Demonstrated resiliency and tenacity in driving solutions for low-income individuals seeking supportive services, including conflict management, influencing without authority, and problem-solving beyond prescriptive solutions
- Demonstrated ability to network, build connections, and develop strong relationships in the community
- Strong understanding of low-income and/or Medicaid populations with complex needs, including individuals experiencing chronic homelessness and behavioral health conditions
- Strong leadership skills with the ability to work independently and with minimal direction
- Strong communication and presentation skills
- Sensitivity to individual needs and a strong person-centered orientation
- Ability to work in a complex matrix environment, influencing others through communication and organizational skills
Preferred Qualifications: - Housing or Affordable housing experience
- Understanding of Medicaid and Medicare programs, as well as managed care
- Bilingual
- Healthcare experience
