This position is mostly working weekends with occasional coverage during the week. Hours are 8:00AM-4:00PM or 9:00AM-5:00PM
RN Patient Navigator Clinical Obs Part Time Days Position Summary
The Patient Care Services Manager serves as the hospital lead for managing daily patient throughput. The primary goals of this position are to improve hospital throughput; increase inpatient bed utilization; monitor appropriate utilization for all levels of care; reduce patient or bed turnaround delays; monitor and reduce LOS/avoidable hospital days; facilitate TEMPO and discharge by Noon. Directs and oversees the process that ensures delivery of patient care in accordance with established hospital, State, Federal and Joint Commission, ANA policies, procedures, regulations, ethics, and standards.
THE RN PATIENT NAVIGATOR CLINICAL OBS PART TIME DAYS CANDIDATE WILL POSSESS THE FOLLOWING EDUCATION, LICENSE/CERTIFICATIONS, AND EXPERIENCE.

POSITION ACCOUNTABILITIES AND PERFORMANCE CRITERIA
1. Serves as the primary hospital throughput Leader for inpatient beds and patient flow, identifying and remedying delays within various patient care services departments (e.g., Inpatient, ED, Surgical and procedural, Imaging, Special Diagnostics, etc.).
- Monitors daily patient discharge disposition for immediate entry into PBAR and reconciling to Case Managers final patient disposition for accuracy.
- Uses bed tracking technology, rounding and communications with staff to maintain an understanding of house-wide bed supply and demand.
- Assures patient discharges entered immediately into PBAR. Works with admission portals (e.g., Emergency Department, External Transfer, and Direct Admissions) and inpatient floors to plan for daily scheduled and unscheduled bed demand.
- Maintains communication with admission portals regarding bed availability and house status.
- Triages observation admissions to ensure appropriate use of inpatient and flexible care areas.
- Partners with caregivers to pull patients from the admission portals to the inpatient floors and reduce wait times for inpatient beds.
- Assures no admit holding in the ED and patients are admitted to the units and entered into PBAR prior to Midnight.
- Works with the physician leadership to prioritize admissions and place patients directly to the most appropriate and available inpatient bed.
- Makes timely decisions regarding patient placement and escalates to the physician leadership as needed.
- Maintains continuous communication with the Nursing Supervisors. Works with Nursing Supervisors to assure the teletracking system is updated timely and consistently.
- Provides leadership in critical thinking, conflict management, and problem solving.
- Recommends alternative approaches to maximize patient outcome potential.
- Anticipates, recognizes and manages emergent/unsafe situations and initiates interventions.

Education/Other Qualifications:
Minimum Education: Bachelor's Degree in Nursing, related field or equivalent competencies/job experience
Preferred: Master's Degree in Nursing, Business Administration, Case Management, Finance or related field.
Minimum Experience: Minimum of 5 years experience in nursing leadership, hospital operations, case management, Finance or related field, clinical related specialty; one year supervisory or management experience, working knowledge of Quality Improvement process and Joint Commission standards.
Required Certifications/Registrations/Licenses: A valid Permanent Multi-State RN License from the state in which you reside is required. Should you hold a RN license from another Compact state, you are required to apply for and obtain a Multi-State RN License from the state in which you reside within 30 days.
