Job Title: Registered Nurse (RN) Job Specialty: Home Health Job Duration: 13 weeks Shift: Day Shift, 5x8 Hour Guaranteed Hours: 40 hours per week Experience: 1 year of nursing experience required License: Registered Nurse License in the state of North Carolina Certifications: Basic Life Support (BLS) for Healthcare Provider certification Must-Have: Driver's License Job Description: - Provide competent clinical nursing care consistent with professional standards. - Report to and be accountable to the Nurse Manager/Director. - Coordinate and deliver patient care utilizing the nursing process in a hospital setting. - Participate in identifying and meeting personal learning needs. - Attend educational programs based on learning requirements. - Use relevant patient education materials. - Assess readiness to learn for patients, families, and caregivers. - Identify expected patient outcomes and revise care plans as needed. - Engage in discussions with the interdisciplinary team to evaluate patient care. - Communicate relevant information to ensure continuity of care. - Demonstrate competence in caring for patients with complex issues. - Conduct bedside point-of-care testing as required. - Follow healthcare organization and nursing standards, policies, and procedures. - Organize and prioritize care based on patient and family needs. - Consult with appropriate resources regarding complex care issues, teaching needs, and discharge planning. - Develop personal goals for professional growth. - Achieve goals within identified time frames or renegotiate with a supervisor. - Participate in intra/interdepartmental collaboration and unit goals achievement. - Promote cost-effective care and provide feedback to coworkers. - Be aware of public policy and regulatory guidelines affecting health care. - Maintain a safe, clean, and secure environment. - Recognize data from complex situations to establish care priorities. - Synthesize assessment data into meaningful information for communication. - Assess and anticipate discharge needs for patients and families. - Develop relationships with families to enhance advocacy for patient and family needs. - Collaborate with the patient and family to prepare or update the plan of care. - Use available multidisciplinary resources in planning care. - Address cultural and ethnic issues in planning patient care. - Prioritize tasks effectively for patients with complex problems. - Read journals for studies or articles applicable to practice and share ideas with staff.
