OverviewThe RN Case Manager assesses, plans, implements and monitors a comprehensive plan of care for her/his group of patients. This is done in collaboration with other members of the medical team, i.e. physicians, social workers, PT/OT staff, pharmacists, respiratory, dietary, and nursing. Maintains a focus on the patients progression of care throughout the hospitalization, working with the care team on the appropriate utilization of resources/services and creating a safe, effective and efficient transition/discharge from the hospital.
QualificationsMinimum Education and Experience Requirements:Licensed RN in Florida. BSN preferred. 2 years recent RN experience (in a clinical capacity) applicable to the designated population served is required. Prior Case Management experience is preferred. Case Managers hired after January 1, 2018 who do not possess a BSN and/or a MSN will be required to obtain, at a minimum, a BSN within three years of employment as a Case Manager. Professional nursing board certification in specialty area preferred. Case Managers hired after January 1, 2018 will be required to obtain specialty certification within two years of employment as a Case Manager.Motor Vehicle Operator Designation:Employees in this position:Will not operate vehicles for an assigned business purposeNOTE: A frequent driver is defined as one who uses his/her personal or Shands automobile a) at least once daily, b) at least five individual trips per week or c) drives, on average, over 150 miles per week in the performance of his/her job.
