Position Information:
The Care Coordinator is a Registered Nurse who is assigned to a specific clinical care area facilitating the plan for inpatient/outpatient care by using a collaborative interdisciplinary approach. The Care Coordinator will focus on anticipation integration and coordination of clinical care and discharge planning services necessary to meet the needs of patients and to optimize clinical and resource outcomes. The Care Coordinator collaborates with the interdisciplinary team in assessing patient needs and developing the plan of care. The Care Coordinator will also act as an advocate for the patient physician and hospital relative to compliance with federal state and third party utilization management regulatory requirements.
Experience Requirements:
3 Years Nursing - Related Area Required - And Clinical - Case Management Or Clinical - Utilization Management Or Clinical - Home or Community Health Preferred
Education Required:
Graduate Nursing Required - And Bachelor's Degree Nursing Preferred
Licensure/Certification Requirements:
Registered Nurse Required

Mon, Tues, Wed 2p to 10p
Weekend Requirements:
