Swing Case Manager
(ONSITE) Santa Ana, CA 92704
Schedule: (Swing) Tuesday - Saturday 3:00p – 11:30p
Pay: $23 per hour
Key Requirements:
Provide comprehensive case management services to underserved individuals.
Conduct intakes, and assessments to determine participants needs and develop individualized service plans (ISP), schedule interdisciplinary meetings.
Connect participants with appropriate resources and services to address barriers.
Collaborate with community partners, shelters, and other agencies to facilitate access to resources and coordinate appropriate care.
Maintain accurate and up-to-date documentation of participant interactions, progress, resources and referrals.
Advocate on behalf of participants to ensure access to essential services and support their goals for housing stability and self-sufficiency.
Participate in team meetings, case conferences, and ongoing training sessions as required.
Qualifications:
Minimum 1 year experience in Case Management, Social Work.
Comfortable working with homeless population.
Strong interpersonal skills and the ability to build rapport with diverse populations.
Excellent communication and problem-solving abilities.
Compassionate and non-judgmental approach to working with clients.
