SUMMARY OF POSITION:
The Family or Adult nurse practitioner is responsible for on-site medical care to the participants in the Mocj transitional for individuals who are released from incarceration.
This role will also require leadership duties involving clinical protocols, testing, vaccination, education, and communication with HW leadership and other agencies including MOCJ, and other duties as needed in collaboration with the Medical Director
The NP will perform a medical assessment of all new admissions and provide comprehensive medical evaluation to patients who present for routine, preventative, acute, and chronic disease care at the site.
The NP will coordinate all of the services necessary for health promotion, maintenance, rehabilitation, and the prevention of disease and disability.
The provider will work closely with and provide guidance when necessary to the nurse supervisors, as well as other nurses, medical assistants, social workers, case managers, and site directors.
ESSENTIAL FUNCTIONS:
Creates clinical protocols in collaboration with the Medical Director
Supervises nurses and medical assistants, in collaboration with the Director of Nursing
Optimizes clinical workflows and troubleshoots problems, in collaboration with the Medical Director and the Director of Clinical Operations
Communicates with leadership from HW as well as with other agencies including DHS, as coordinated by the Medical Director
Provides clinical teaching to staff
Performs duties with professionalism and compassion using an evidence-based and harm reduction approach
Provides direct patient care: assesses and interprets medical and psychosocial histories; performs physical examinations (including gynecological and breast examination as appropriate); identifies problems; formulates a plan of treatment; provides education and referral (as indicated); and evaluates the effectiveness of the interventions prescribed
Consults with or refers clients with abnormal conditions or other medical, psychological or social problems to the collaborating physician or other appropriate referral course when indicated
Performs or orders laboratory tests and interprets the results as part of health management. Reports communicable diseases in a timely manner to the appropriate Health Department as per health center protocol
Orders medications
Follows established treatment guidelines as indicated by Health Center protocols
Conducts health teaching and counseling during the medical encounter, as appropriate, enabling the client to make informed choices about health and treatment options
Cover for other NPs who are unavailable (PTO, illness, etc.) whenever possible
Share call on a rotating basis with other NPs
Completes the client's clinical record documentation in a timely manner specific to the applicable policies and procedures of the health center
Understands the medical emergency procedures and is familiar with the location of the emergency kit/supplies in all clinic settings
Leads the quality improvement/assurance activities
Participates in continuing medical education activities to enhance professionalism and to ensure quality of care
Provides on-call coverage as scheduled.
Works collaboratively with all members of the health center to build a supportive team.
Applies infection control measures according to Health Center Policies and Procedures.
Maintains professionalism and adheres to client confidentiality.
EDUCATION/EXPERIENCE/QUALFICATIONS:
2 to 4 years of experience as a Nurse practitioner
Experience and knowledge of managing clients with substance abuse preferred.
Licensed to practice as a NYS Nurse Practitioner.
Licensed and currently registered as a Registered Nurse in the State of New York.
Previous experience with an electronic health record, preferred
Visual acuity and manual dexterity to interface with the computer and perform medical procedures as indicated
Auditory acuity to handle phone calls and interact extensively with patients
Maintains CMEs/CEUs required for licensure
Maintains CPR certifications
Maintains Infection Control certification.
Maintains DEA registration.
Certified by ANCC or AANP
Bilingual English/Spanish preferred, but not require
