We are seeking a dedicated Nurse Practitioner (NP) to join the Independent Assessor Program under the NY State Department of Health. This role involves conducting thorough assessments and reassessments to determine eligibility for managed long-term care services. The NP will work closely with consumers, reviewing charts and conditions to confirm their ability to receive care at home rather than in a nursing home.
Key Responsibilities:
- Conduct 3-hour assessments and reassessments with consumers.
- Review and compare charts to assess consumer eligibility for home-based care.
- Collaborate with Physician Assistants (PAs) and other medical professionals.
- Ensure the entire process is completed within 11 days of the consumer's request.
- Complete necessary training, including 3 sessions, each lasting 3 hours.
Qualifications:
- Active membership in Medicaid is mandatory.
- Experience in managed long-term care services preferred.
- Ability to work across 62 counties in NY State, with a focus on Nassau, Suffolk, Long Island, Albany, Erie, and Westchester.
