Role: Health Information Specialist
Location: San Jose, CA - Onsite (Need locals)
Description:
Will be working on unbilled orders; calling doctors offices to get them to fax orders that are signed and dated.
Maintains the electronic health record by analyzing medical records for completeness. Determines deficiencies and notifies providers to complete records. Ensures accurate and timely filing and chart order of medical records as well as maintenance and retrieval of medical records and reports. Creates and routes chart folders for new patients, collects all current discharges and old charts from various departments. May work with Physicians regarding incomplete charts. Adheres to all standards regarding patient confidentiality; rules of conduct as outlined in policy; and departmental rules, policies and procedures. JOB ACCOUNTABILITIES: CHART COMPLETION.• Pulls and files high volumes of patient records and documents.• Merges duplicate patient health records in EPIC , Legacy system and on paper.• Reports duplicate /registration error trends for follow-up.• Accurately collects current and old records for discharged patients from nursing units by the end of the shift.• Delivers charts to patient care areas within specified time frames or as needed.• Accurately sorts loose reports within specified time frames and routes to appropriate charts.• Accurately sorts, distributes and files transcribed reports within established time frames.• Accurately files patient charts and folders within established time frames.
PHYSICIAN COLLABORATION
• Notifies physicians of incomplete records and pulls charts for physician completion.• Retrieves incomplete records for physicians.• Logs all physician visits.• Checks records for completion and updates chart control system after completion by physicians.• Suspends physicians for delinquent records.• Accurately updates potential suspension list on a daily basis.• Promptly notifies all departments of physicians removed from suspension.• Maintains and updates listing of physicians receiving letters.• Accurately maintains suspension log and list.• Mails copies of physician reports weekly• Files copies of physician reports daily in mailroom.• Pulls charts in a timely manner.• Accurately performs reception duties which include filing reports into in-processing areas, retrieving charts within established time frames and promptly assisting physicians or visitors coming into the department.• Properly evaluates requests for patient information and applies appropriate release of information guidelines.
EDUCATION:
Equivalent experience will be accepted in lieu of the required degree or diploma.HS Diploma or equivalent education/experience
SKILLS AND KNOWLEDGE:
Knowledge of Medical terminology, chart analysis, medical record tracking, abstracting. Verbal communication and telephone skills. Ability to deal effectively with detailed confidential information. Knowledge of Windows and Microsoft Office applications experience. Ability to read and comprehend complex instructions, correspondence and medical record documentation. Development of good working relationships with physicians, clinical ancillary departments, department coworkers, and other health care professionals. Knowledge of Electronic Health Records system applications
