Role Overview: As a Case Manager, you will be responsible for supporting 15 to 25 clients at a time, who are homeless veterans or their families. Your role will be to provide comprehensive support, acting as a guardian for these individuals. Responsibilities include helping clients secure housing, preventing homelessness, assisting with obtaining a driver’s license or Social Security benefits, and managing their basic daily living needs.
Key Responsibilities:
- Assist clients in transitioning from homelessness to stable housing.
- Support clients with essential services and benefits.
- Conduct weekly check-ins with clients.
- Facilitate transportation to appointments and housing checks using a company vehicle.
- Ensure timely and accurate reporting and documentation of client progress.
Requirements:
- Valid driver’s license with a clean driving record (less than 3 points).
- Ability to pass a federal background check, fingerprint test, and drug screening.
- Strong preference for candidates who are veterans.
