Summary:
The RN – Team Coordinator, under the supervision of the clinical nursing leader or department manager, he/she is responsible and accountable for the care of the assigned patients and directs others to assure compassionate and patient centered care is completed on a shift to shift basis. The RN is expected to practice autonomously within the scope of professional standards, the rules and regulations established by the Tennessee or Virginia Board of Nursing, and knowledge of the policies and procedures within Ballad Health. He/she demonstrates skill in developing and guiding collaborative collegial practice that delivers safe competent care based on meticulous application of the nursing process. He/she demonstrates sensitivity and understanding of the diverse cultural backgrounds of patients and caregivers. The team coordinator works directly with staff on the unit to ensure compliance with patient care and service problem resolution. The position involves maintaining current knowledge of evidence based nursing practice and the ability to demonstrate critical thinking skills in the implementation of all phases of the nursing process. He/she actively participates in patient care quality and performance improvement through data collection, aggregation and analysis of relevant key indicators of quality (KIQ's). Participates in LEAN initiatives for the improvement of processes and patient care delivery. This commitment to quality results in the development and implementation of action plans to include the monitoring of the effectiveness of improvements. Must have knowledge of nursing processes and evidence based practice that constitutes the care given in an acute care hospital setting. The RN serves as a professional role model as a caregiver, mentor, teacher and resource for peers and other providers. He/she demonstrates competency requirements for the Registered Nurse in employed clinical department and for the specific patient population served. RN is responsible for the nursing care of patients assigned to him/her.
The Team coordinator assists with patient assignments, planning, organizing, evaluating, and supervising nursing care within the department. Facilitates communication within the department and interdepartmentally. The Team coordinator identifies conflict at the unit level and initiates resolution and/or escalates through the chain of command as indicated. The Team coordinator provides direction and support for nursing staff. The Team coordinator serves as role model for clinical and ethical practice within the department. The Team coordinator is responsible for education and implementation of patient centered care and department/organization initiatives. Plans and conducts a minimum of two hours of job related educational programs. Verbalizes understanding of Shared Governance Model and willingly serves on at least one Shared Governance, hospital committee, task force, or project to improve patient care or work area.
Individual must possess excellent interpersonal communication skills and function as a liaison between physicians, interdisciplinary treatment team, patients, and families. The person in this position must be able to work as a team leader by communicating effectively with team members and providing an open dialogue for solutions. It is of the utmost importance that written communication is legible.
This individual serves as resource and clinical expert for mentoring staff, team development, and nurse retention. Responsible for mentoring new staff and new graduate nurses either individually or through an assigned preceptor. Monitoring and reporting to clinical leadership trends that might indicate an individual team member is having difficulty performing role expectations. The team coordinator is expected to set an example for all team members of commitment and loyalty to Ballad Health by openly supporting and participating in hospital projects and events.
The person must be committed to continuing education by staying abreast of current medical/healthcare information and by applying evidence-based practices in areas of responsibility in order to remain competent in the ever changing role of the position and development in health care.
Serves as a liaison to System Clinical Educators – ensuring new clinical processes are adopted and provides feedback to clinical education if concurrent training requires revision or additional support.
Teaching colleagues/peers and serving as an effective role model is a basis of professional practice. The Team coordinator demonstrates compliance with established Ballad Health policies, dress code, and standards of professional conduct. Confidentiality is essential to the role.
Individual will assist the department leader in interviewing and selection of team members, and provide input into competencies and evaluation of team members.
Requirements:
College degree required. BSN preferred. Health care related Bachelor's degree or associate nursing degree required. Preferably the RN with an Associate Degree will be actively pursuing a bachelor's degree from an accredited school. Minimum of 2 years of relevant clinical experience required. One to three years of experience in an acute care setting preferred. A relevant background in the clinical position applying for is preferred. Supervisory experience preferred. National Professional Nurse certification is desired. One year charge/management experience preferred.
Licenses and Certifications: Valid and active RN license in the appropriate state for practice. AHA BLS certification is required. ACLS certification obtained within one year of assuming the position and maintained continuously.
