*Responsibilities:*
· On an ongoing basis, assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment.
· Completes an individualized initial plan of care based on immediate needs and completes this documentation within the required timeframe.
· Completes admission paperwork in an efficient and timely manner ensuring that the caregiver/patient understands the information provided.
· Helps to ensure seamless service delivery as a member of the social work staff. In addition to admission/referral responsibilities, assists with emergency visits and routine visits as necessary to meet the needs of patients and caregivers.
· Provides anticipatory grief counseling services to patients and their caregivers according to the established plan of care and as ordered by the physician.
· Helps patients/caregivers in understanding the philosophy of hospice and the dying process.
· Carries out psychosocial evaluations and plans interventions based on evaluation findings. Counsel’s patient and family/caregivers as needed in relationship to stress and other identified coping difficulties.
· Provides leadership in strategic planning including identifying opportunities for additional or improved services to address unmet customer needs.
· Provides advocacy and assists patients/caregivers with identifying community resources for nursing home placement, funeral home planning, etc.
· Assists in managing discharge planning and processing when appropriate. Assists patient and family/caregiver with planning for end of life needs.
*Qualifications:*
· Current MSW licensure for the state.
· A graduate of a Master's program in social work accredited by the Council on Social Work Education preferred.
· Minimum of one year of experience in health care, with hospice experience preferred.
· Understands hospice philosophy, and issues of death/dying.
· Demonstrates good verbal and written communication, and organization skills.
· Demonstrates strong public relations skills.
· Demonstrates autonomy, organization, assertiveness, and flexibility and cooperation in performing job responsibilities.
· Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Must be able to travel throughout the service area.
