*FAMILY NURSE PRACTITIONER*
The Family Nurse Practitioner (FNP) shall report to the Medical Director of IHCWV. The Nurse Practitioner, through collaboration with a physician, shall provide primary health care to individuals in our serving area within the parameters of the West Virginia Board of Nursing “Code and Legislative Rules for Registered Professional Nurses.” While in this position, you will play a significant role in the preventive care in individual patients, families and communities by forming strong, trusting, long-term relationships. Through a one-on-one dialogue, you will have the opportunity to empower individuals to make better health choices and lifestyle changes to prevent chronic disease by promoting health advocacy and education that goes beyond the exam room and by preserving health instead of simplifying treating an illness.
*Specific duties and responsibilities will include the following:*
1. Assess, diagnose, and treat common health problems for persons of all ages and evaluate response to treatment of common health problems for persons of all ages; obtain medical histories and perform physical examinations.
2. Order and interpret select diagnostic studies; prescribe or recommend medications and other treatments.
3. Promote positive health behaviors and self-care skills through education and counseling; arrange for referral to other health professionals as appropriate; collaborate with physicians and other health professionals as needed.
4. Establish and document health care plans and diagnosis, ensure records are sufficiently complete to ensure the effectiveness of each plan.
5. Complete patient charts in timely manner to ensure continuity of care is accurate at all times.
6. Attend continuing education offerings; maintain required licenses and certifications.
7. Perform comprehensive physical and psychosocial assessment including health maintenance screening and risk evaluation. Identify behaviors and environmental factors affecting health/illness.
8. Exhibit skill, creativity and initiative in contributing to the improvement of overall delivery of care, customer service, and patient and family satisfaction levels through active participation in Provider/Management meetings.
9. Other duties as assigned by the administrator.
*QUALIFICATIONS:*
1. Masters degree in nursing or higher and certification as a Nurse Practitioner by a national certifying organization (required), 2+ years of experience as an NP (preferred)
2. Current Registered Nurse Licensure by the West Virginia State Board of Nursing.
3. Has applied for or maintained West Virginia prescriptive authority.
4. ANCC or AANP certification for Nurse Practitioner-Adult NP
5. Proficiency in Microsoft Office: Excel, Word, Access, PowerPoint; Internet navigation, required
6. Ability to communicate verbally and in writing effectively with diverse audiences with varying levels of education, training, and learning styles.
7. Demonstrated ability to complete initial and ongoing client assessments (clinical and diagnostic reasoning), care planning, case management, and evaluation of patient compliance in self-care and other related outcomes coupled with providing nursing care.
8. Current provider card in Basic Life Support (BLS) and Advanced Cardiac Life Support

Monday - Firday 7:30-4:00
Rotating Saturday shifts: 8:00-12:00
