*Nurse Practitioner - Summary:*
We are currently searching for a full-time dedicated and compassionate *Nurse Practitioner* to work directly with a MD within a group setting to provide direct patient care to Skilled Nursing & Assisted Living facilities and Rehab patients. You will work directly with an attending MD at the facility. The *Nurse Practitioner* will follow treatment plans set forth by hospital discharge summaries and the attending MD. The goal is to deliver personalized quality post-acute and long-term care. You will serve as the front line of care and deliver the gold standard of care set forth by Physicians Services Group (PSG) of South Carolina.
*Nurse Practitioner - Responsibilities:*
* Deliver patient care based on obligations set forth by the State of South Carolina permissible under their license
* First line provider at the post-acute care and represents PSG at the facility
* Evaluate and treat new admissions, follow-ups, scheduled routine Medicare follow-up based on PSG guidelines, as well as, deliver acute care daily
* Liaison with primary care and specialist to help coordinate the continuum of care
* Update family members on patient status
* Work directly with facility to maintain compliance with appropriate paper work for PT/OT/Speech and Social Services
* Coordinate and execute DC planning with social worker
* Review all daily lab work and radiology reports ordered by PSG
* Work directly with supervising MD to deliver quality care
* Represent PSG in a professional business manor
* Solve acute problems for PSG patients and Management at their facilities
* Responsible for daily documentation within the EMR
* Responsible for daily E/M billing based on guidelines/expectations/opportunities set forth by PSG guidelines
* Work in a professional, approachable and cordial manor
* Create a partnership between care coordination/oversight and care delivery at the facility
* Participate in improvement initiatives set forth by PSG
* Work directly with Medical Office Coordinators to help organize and deliver quality care set forth by PSG guidelines
* Maintain appropriate census set forth by PSG guidelines
*Nurse Practitioner - Qualifications:*
* ANCC certification or equivalent
* SC license preferred
* Registered Nurse certification
* Perform duties within *M-F 8a-4p*
* Passion to deliver quality care and to help make a difference in the post-acute setting
* Must have a strong clinical background and experience in hospital or acute setting preferred
* Must have strong interpersonal skills, be a team player, and the ability to work independently
* Advanced medical knowledge of common chronic disease states
* Must be able to recognize and solve acute problems and treat appropriately
* Previous experience with EMR and basic computer skills required

Pay: $90,000.00 - $120,000.00 per year
