The LCSW is an integral member of the *Dialysis center,* interacting with all members of the care team. We are a team of *highly skilled and professiona*l staff that are committed to providing *exceptional care *and support to our patients and their families.
*Responsibilities *The Social Worker is responsible for meeting the non-medical needs of the patients and the patients' families, thereby *assisting the patients* to achieve a more adequate, satisfactory and productive lives. Each patient is interviewed by the Social Worker at the time of admission to *access the patient's social needs* to assist in developing a *patient care plan*. Progress notes will be maintained on a regular basis. The Social Worker will participate in *Care Plan meetings* with attending Physicians and the appropriate medical team. The goal of the Social Worker is to *work collaborativel*y with the *patient and the family *to help them cope with the personal, emotional health and environmental needs. Assisting with *travel *and *transportation*, coordinating *hospital admissions* and discharge plans, and providing information regarding the appropriate *utilization of community resources* to meet the needs of the patients.
*Qualifications *Must have completed a course of study with specialization in a clinical practice. Holds a Masters degree from a graduate school of social work accredited by Council on Social Work Education. *We will consider an Associate Clinical Social Worker* that will be required to work under the direction of our L.C.S.W.
