Responsibilities
* Function autonomously to care for patients with complex, acute, critical and chronic health conditions.
* Prescribe medications including the frequency and dosage
* Work individually and in interdisciplinary teams of resources and consultants
* Facilitates patient transition within and between health care organizations to ensure accurate communication among the health care providers
* Record and maintain patient records in timely manner to ensure all services are accurately accounted for and available for review by the interdisciplinary teams
* Talk to patients about effectively managing their health, as well as assisting in designing appropriate treatment plans and recommending further care

* Demonstrated problem-solving and workflow management
* High level of clinical proficiency and excellent decision-making skills.
* Ability to work independently and as an effective member of a healthcare team.
* Excellent oral and written communication skills.
* Ability to effectively collaborate and build relationships with patients, families and all care providers
* Proven organizational skills and ability to prioritize effectively.
* Strong computer skills with knowledge and experience with Electronic Medical Records
Qualifications
* Board Certified as a FNP
* DEA Substance License required
* BCLS/ACLS certification required
* Prior APN experience preferred
Job Type: Part-time
Pay: $100.00 - $150.00 per hour
Expected hours: No more than 10 per week
