ChristianaCare Hospital Care at Home Program is hiring a Full-time registered nurse to work in the home taking care of med-surg patients.
The in-home RN cares for patients within the scope of the nursing practice license. Leveraging a variety of technology tools and software, the RN is responsible for the clinical monitoring and care for the patient including, but not limited to: clinical assessment, evaluation of clinical information that is being monitored and collected from patients, managing clinical protocols, assessing psychosocial needs, recommending services for the patient to the provider (i.e home health, DME, rehab services), creating collaborative care plans, performing case management functions and escalating clinical concerns to a provider in accordance with established protocols.
Our Hospital Care at Home Program enables patients to receive 24/7 hospital-level care from the comfort of their homes rather than in a hospital.
