Coordinator, Referral & Plcmnt
Job Summary: Responsible for responding to crisis calls for the community and persons whose lives are disrupted or complicated by mental illness, behavioral disturbances, substance abuse issues or inability to function or maintain in the community. Provide professional and clinical triage services to mental health and substance abusing clientele, to include dispatch of evaluators, mobile crisis, and inpatient placement to external and internal facilities. Works as part of a multidisciplinary team to provide placement, referral, admission, intervention or screening, through primarily phone contact with some in person communication.
Essential Functions
- Is the point of contact for the client and assists with identifying and connecting client with immediate and ongoing needs; maintains working knowledge of community resources and acts as a broker between clients, the community and caregivers.
- Works as part of a multidisciplinary team to provide placement, referral, admission, intervention, or screening, through primarily phone contact with some in person communication.
- Monitor clients in the crisis waiting area and update on placement status
- Manage Diversus Health Emergency Response communications
- Facilitate client-centered internal provider huddles, to review client needs and concerns; provides feedback to client and treatment team and participates in treatment planning review.
- Facilitate ongoing communication with client’s internal and external service providers (emergency departments, family, schools, inpatient facilities) to ensure client care is coordinated effectively and efficiently.
- In clinical settings, works in collaboration with the clinical team to assess clients in crisis and to de-escalate clients when necessary
- Obtaining initial ATU insurance pre-authorizations
Knowledge/Skills/Abilities
- Bachelor’s Degree in a behavioral health or healthcare related field or equivalent experience.
- One year of volunteer or professional experience in the behavioral health field, preferred
- Desire to work with people who have mental illness
- A thorough understanding of systems of care and integration of physical and behavioral health
- Adapts to differences in organizational cultures (primary care, substance use, social services, etc.)
- Ability to take initiative and use critical thinking to solve problems independently
- Excellent verbal and written communication skills and can easily build rapport
- Proficient in Microsoft word and Excel and can type at least 25+ w.p.m.
