Physical Therapist - Outpatient - (PT)
StartDate: ASAP
Available Shifts: 8 D
Pay Rate: $1739.28 - $2003.95
2 years CURRENT PT experience - Required (New grads will not be considered for this need)
o Must be flexible - Will need to work Inpatient PT as needed
o Must have VERTIGO experience (LSVT BIG Certification preferred)
o Graduate from an accredited school of Physical Therapy - Required
o Previous Rehab Experience - Required
o Previous travel experience - REQUIRED - At least 3 mos.
o CPSI EMR experience - Highly Preferred
o Previous experience working with PTA's - Preferred
o This is a small community without public transport. Traveler candidate must have their own vehicle. This is a firm requirement.

o Work History
o Skills Checklist (pertinent to position), current to 1 year
o 1 Supervisory Signed Reference (from within the past two years of employment)
o License/Certifications - MUST be uploaded with profile (NBSTSA, BLS - Facility will not pend license/certs)
o BLS (AHA)
