POSITION SUMMARY
The Physical Therapist (PT) is a professional member of the health care team who provides Physical Therapy services to patients in compliance with all applicable practice acts. The Physical Therapist performs duties within standards of clinical practice as defined by the Department of Professional Regulation, the American Physical Therapy Association, and the policies and procedures of the agency, and functions under the direction of a physician and nursing supervisor in the deliverance of skilled Physical Therapy care to the client.
RESPONSIBILITIES
- Provides initial and ongoing comprehensive assessments of the client’s needs, including Outcome and Assessment Information Set (OASIS) assessments.
- Establishes a treatment plan in consultation with the physician and revises it with the approval of the physician as necessary.
- Assists in the development and implementation of the interdisciplinary client care plan as it pertains to Physical Therapy.
- Teaches supervises and counsels the family and client in the physical therapy program.
- Supervises Physical Therapist Assistants at least every 30 days.
- Communicates with the physician whenever changes occur regarding the client’s status and obtain additional orders, if necessary.
- Communicates with other team members (including Home Health Aides) and, when appropriate, instructs them in certain phases of physical therapy that they may use while working with the client.
- Writes clinical/progress notes on the date of the visit and incorporates same in the client's record according to company policy.
QUALIFICATIONS
- Graduate of an accredited Physical Therapy program or equivalent education.
- Licensed to practice as a PT in the State of Florida.
- Current BLS Certification.
- Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
- At least one year of experience as a Physical Therapist.
- Home healthcare experience is preferred but not essential.
- Electronic medical record experience is preferred but not essential.
- Must display good emotional health and be able to physically tolerate much standing, stooping, bending, and heavy lifting; must meet all agency regulations.
- Must be self-directed and able to work with little supervision and have good observational, judgment, and communication skills.
