Job Summary
The Senior Director for Enhanced Care Management (ECM) and Behavioral Health Services will lead a team providing housing and behavioral health services as part of UWCCR's CalAIM expansion. This role oversees both clinical and non-clinical staff, ensuring high-quality service delivery, compliance with regulations, and effective program management. The Senior Director will also drive program development, budget oversight, and strategic partnerships with community providers and stakeholders.
Key Responsibilities
Program Leadership & Development (60%):
· Vision and Strategy: Develop the overall vision for Enhanced Care Management programs, ensuring alignment with UWCCR's mission and CalAIM goals. Identify strategic opportunities for growth and innovation in program delivery and participant services.
· -Service Delivery Oversight: Oversee the planning, coordination, and execution of housing services, including housing location, housing counseling, and retention programs. This includes developing comprehensive service delivery models tailored to meet the needs of diverse populations, such as those experiencing homelessness or behavioral health challenges.
· Quality Assurance and Compliance: Ensure that all services comply with contractual agreements and regulatory requirements. Regularly review service delivery standards to identify areas for improvement and implement quality control processes to maintain high standards.
· Budget Management: Develop and manage program budgets, overseeing all financial aspects of ECM, including expenditures, financial reporting, and ensuring funds are allocated effectively. Adjust budgets as needed to accommodate program changes or growth while staying within financial limits.
· Grant Writing and Fund Development: Assist the executive team in responding to RFPs and writing grant proposals. Identify opportunities for additional funding to support program expansion and development. Build relationships with funders and ensure timely reporting of program outcomes.
· Community and Stakeholder Engagement: Develop and maintain partnerships with local service providers, community organizations, and government agencies. Facilitate collaboration between UWCCR and other organizations to improve service integration and enhance outcomes for participants.
· Program Expansion: Identify gaps in service offerings and collaborate with executive leadership to design new programs or expand existing services. This includes exploring innovative approaches to behavioral health and housing services.
· Health Service Integration: Work closely with internal and external healthcare providers to develop a comprehensive and integrated care model that addresses the housing and behavioral health needs of participants.
Team Management & Supervision (30%):
· Staff Supervision: Directly manage clinical and non-clinical staff, including therapists, counselors, housing specialists, and case managers. Provide clear leadership and set expectations for staff performance, ensuring alignment with program goals and UWCCR values.
· Performance Coaching: Offer ongoing coaching and feedback to staff, setting clear objectives for performance improvement. Regularly evaluate staff performance through formal reviews, appraisals, and individual check-ins, providing guidance for professional growth and development.
· Training and Development: Develop training programs for team members, focusing on evidence-based care management and behavioral health interventions. This includes conducting formal training sessions and providing informal mentorship in areas such as motivational interviewing, care coordination, and behavioral health treatment plans.
· Collaboration and Conflict Resolution: Promote a collaborative working environment by fostering open communication and teamwork. Effectively resolve conflicts and mediate disagreements among team members to maintain a positive work culture.
· Clinical Oversight: Provide clinical supervision for licensed and non-licensed staff, ensuring they are equipped to manage complex cases involving mental health and housing needs. Oversee case conferences, guiding staff in developing individualized care plans (ICPs) for participants.
· Workforce Planning: Assist in the recruitment, hiring, and onboarding of new team members. Plan for workforce needs, ensuring adequate staff coverage to meet program demands, especially during periods of increased workload or staff absences.
· Crisis Management: Ensure staff are trained to handle crises, including mental health emergencies and challenging situations with participants. Provide guidance and leadership during critical incidents and debrief with staff afterward.
ECM – CalAIMS– (10%):
- Familiarity with Screening Tools: Become familiar with The Accountable Health Communities Health-Related Social Needs Screening Tool.
- Screening Review: Review Accountable Health Communities Health-Related Social Needs Screenings.
- Eligibility Diagnosis: Approve eligibility diagnosis Z codes in relation to the Accountable Health Communities Health-Related Social Needs Screening.
- Clinical Needs Assessment: Address clinical needs associated with eligibility determination.
- Staff Support: Support staff in the development of clinical plans.
- Program Development: Provide direction in the development of programs involving LCSWs.
Qualifications:
Required:
· Master’s Degree in Social Work (MSW) or Marriage and Family Therapy (MFT).
· Active, unrestricted LCSW license issued by the California Board of Behavioral Sciences.
· Minimum of 5 years of experience in program management, preferably in nonprofit, health, or social services sectors.
· Strong leadership skills with experience managing multidisciplinary teams.
· Excellent communication skills, including the ability to engage with community partners, funders, and clients.
Preferred:
· Experience with housing programs and behavioral health services.
· Familiarity with California’s Cal AIM initiative and the nonprofit healthcare landscape.
· Demonstrated success in program development and grant writing.
Skills

Physical & Other Requirements
Mobility: Ability to work in an office environment for extended periods of time. Must be able to move within and between office locations and adjust workspace as necessary.
Lifting: Ability to lift, carry, and transport items weighing up to 40 lbs., such as binders, files, office equipment, or presentation materials.
Travel: Regular travel required within the region to attend meetings, trainings, and site visits. Must have a valid California driver’s license and reliable transportation.
Work Schedule: Flexibility in working hours, including availability for evening and weekend events, meetings, or emergencies, as required by the position.
Remote Work Policy: This position follows a hybrid remote work policy, with the ability to work from home part of the time, balanced with in-office or on-site work as needed to support program and team needs.
Communication: Frequent use of standard office equipment, including telephone, computer, email, and videoconferencing. Must be able to engage in in-person and virtual communication effectively.
Concentration & Decision-Making: Constant need for focused attention to detail and decision-making in a fast-paced environment with competing deadlines and tasks.
Adaptability: Must be able to work independently, manage multiple priorities, and adjust to changing conditions or demands.
Special Requirements
Possession of an appropriate California operator’s license issued by the State Department of Motor Vehicles, and adequate automobile insurance. Possession and maintenance of a good driving record (as evidenced by freedom from multiple or serious traffic violations for at least a two-year duration) that does not contribute to an increase in UWCCR insurance rates; or reliable and punctual transportation to and from various work sites and locations.
