Job Description
- Assesses the physical and psychosocial health status of the client through health and developmental history taking and physical examination.
- Manages the care of routine patients and evaluates the findings of the health assessment in collaboration with physicians, and other health professional to make a prospective decision about treatment, and record findings on the appropriate chart.
- Differentiates between normal and abnormal findings in the screening assessment to detect any changes in health status that require intervention. Analyses and interprets the findings on the H & P.
- Manages illnesses and diseases including prescribing medications and providing care according to protocols and clinical privileges approved by the medical board and nursing services. Diagnoses or treats acute and/or chronic health problems. Creates patient care plans for disease treatment and prevention.
- Provides anticipatory guidance, advice, and counseling to all patients in need, including the families, about diet, hygiene, growth, development, safety, accident prevention, etc. Documents the patient's progress on the chart, including any patient management.
- Orders, performs, interprets, and discusses laboratory tests and procedures with patients.
- Determines possible alternatives in the provision of health care, and makes referrals as necessary.
- Provides health teaching to patients and their families.
- Attends interdisciplinary meetings, conferences, and continuing education programs.
- Develops and submits reports as required. (i.e. infection control)
- Participates in Quality Assurance activities.
- Adheres to infection control policies and procedures.
- Demonstrates Jean Watson's Caring Theory in each patient contact to promote health and well-being.
- Demonstrates knowledge of fire and safety standards and maintains a safe practice environment.
- Demonstrates knowledge of ordering tests, x-rays, mammograms, bone scans, and other radiological studies while following up and managing any abnormal results in a timely fashion.
- Demonstrates knowledge of chemotherapy drugs and radiation therapy as well as management of side effects and adverse reactions.
- Demonstrates knowledge of ordering narcotics/controlled substances.
- Obtains informed consent.
- Demonstrates knowledge of venous access devices, intraperitoneal catheters, and hepatic artery pumps.
- Performs and interprets EKG.
- Adheres to CDC guidelines for patient care management.
- Participates in Continuing Education, Grand Rounds, Tumor Board and In-service Education Program to keep abreast of health trends and current information related to Oncology Care.
- Maintains competence through attendance at continuing education programs.
- Assure policies & procedures related to the Corporation's privacy and security practices.
Hematology
- Patient history and physical examinations
- Preparing patient charts in advance of patient visits (EMR)
- Clear documentation and communication of the plan of care to the care team
- Appropriate documentation and billing of visits and care provided
- Clear and timely communication with patients
- Ability to independently assess patients and develop treatment plans
- Write orders using the EMR
- Interpretation of clinical records
- Follow-up plans and survivorship care
- Participation in tumor board conferences and cancer committee
Minimum Qualifications
- A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and
- Current certificate as a Nurse Practitioner issued by the NYSED; and
- Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
Department Preferences
Departmental Preference: Strongly prefer National Board Certification in the specialty in which you are assigned.
