Position Summary
The Clinical Manager will assume overall responsibility for the implementation, delivery, coordination, and supervision of home health services provided on a day-to-day basis. Serves as a clinical and managerial resource for problem solving and the daily delivery of quality patient care. Extensive knowledge of Medicare, State operations and other regulatory guidelines are required. This position supports established goals and objectives for home health services that meet and promote the standards of quality care and organizational philosophy.
This position is located in our Crestview office.
Essential Duties & Responsibilities
- Serves as a member of the leadership team for Peoples Home Health.
- Supervises clinical and patient care coordinators and a multi-disciplinary team of RN's, LPN's, Therapists, Social Workers, and CNA's ensuring the daily delivery of quality patient care.
- Responsible for monitoring and managing quality assurance data and performance improvement activities to assure compliance with all State, Federal and accreditation guidelines.
- Selects, orients, and assigns work for direct reports. Participates in the development of performance standards and elements of the Performance Management process. Makes recommendations for staff recognition, promotions, disciplinary actions, and terminations.
- Acts as a clinical resource for clinicians and a liaison between Intake Dept, Business Development Team and Patient Care Coordinators.
- Screens admissions, recertifications, and reassessments for appropriate plan of care.
- Monitors staff availability and ensures adequate coverage for patient assignments.
- Possesses strong clinical skills to oversee the assessment, implementation, and delivery of clinical treatments and procedures in a home environment.
- Assists with the interview and onboarding process for new team members.
- Reviews and approves physician orders and patient plan of care for clinical team members ensuring timeliness and accuracy.
- Monitors and evaluates the delivery of quality patient care by interdisciplinary case conferencing, review of documentation, and daily monitoring scheduling of personnel.
- Validates clinical documentation and visit notes are complete and reflects the skilled care provided to receive reimbursement for services provided.
- Demonstrates strong knowledge of Medicare Conditions of Participation, State regulations and CHAP accreditation process.
- Responsible to oversee and ensure customer satisfaction on a daily basis. Identifies issues, resolves, and tracks problems and complaints to ensure a timely resolution.
- Participates in all State and CHAP surveys for accreditation and/or recertification.
- Performs on-site/field visits for staff education and the supervision of new hires and existing clinicians for observation of clinical performance and training.
- Assists in annual training and skills lab to ensure clinical staff are competent and complete timely.
- Coordinates quality improvement activities and staff education for new and existing clinicians/team members.
- Participates in clinical administrative on-call rotation according to needs of the agency and assists with creating, updating, and tracking schedule.
- Knowledgeable of all policy, procedures, and reimbursement guidelines. Provides staff education regarding safety, infection control and overall compliance per OSHA, AHCA and CHAP regulations.
- Assists DON/DCS in the maintenance of agency policy manuals and reference materials for staff.
- Maintains rapport and cooperative working relationship with patients, families, co-workers, business development team, physicians, and external referral sources/vendors.
- Represents the Agency in a professional manner and serves as a positive role model to foster the integration of home health care into the community. Participates and works closely with community affairs and events to increase awareness and referral base.
- Participates in development of specialty programs, staff in-services and continuing education programs to ensure clinical competency.
- Maintains an understanding of the company's scope of services and promotes the company mission, vision, and organizational goals.
- Demonstrates excellent communication skills and understands the infrastructure and organizational structure of the company.
- Promotes a customer friendly atmosphere for all visitors and ensures patient confidentiality and HIPPA guidelines are followed.
- Participates in emergency preparedness activities and the development and implementation of the CEMP.
- Attends and participates in all mandatory staff meetings, State of Affairs and leadership meetings. Serves on companywide committees and PI projects as requested.
- Performs other duties as assigned.
Requirements:
Education/Experience
- Graduation from an accredited RN program. BSN preferred.
- Minimum of 5 years RN experience in a Supervisory role
- At least two years Home Health Experience.
Licenses, Certifications and/or Registrations
- Current RN State of Florida licensure.
- Current driver's license and valid auto insurance.
- Current CPR certification.
