Description of Responsibilities
The Home Health Clinical Supervisor is responsible for managing the daily clinical operations of the home health agency. The HH Clinical Supervisor is responsible for the management and clinical education and support of field staff, and ancillary clerical staff to ensure that patients receive appropriate and safe care, that issues that arise during the day are resolved and that coordination of care between team members is achieved.

Responsibilities include the following:
1. Supervises all field staff
2. Performs field staff evaluations in a timely manner per the agencies policies and procedures
3. Responsible foreducating and maintaining aggregate records for employees from onboarding, orientation, competency, initial field visits and EMR training.
4. Triages calls from field staff, referral sources, physicians and their office staff and other outside entities involved in patients’ care, to ensure the safe and effective coordination of care
5. Serves as a resource for nurses and pharmacists regarding information about home health and home infusion services
6. Ensures nursing services are provided per accepted standards of practice.
7. Mentors and directs field staff as needed
8. Reports any issues that may involve agency liability immediately to the Director of Patient Care Services.
9. Documents QA indicators on an ongoing basis, specifically patient/ referral source/ physician complaints, patient infections, falls, medications errors, and incident reports.
10. Documents significant conversations and events in the patients’ medical records.
11. Participates in the afterhours on call as requested.
12. Serves on agency committees as directed
13. Participates in projects as requested.
14. Attends in-services to enhance knowledge level related to the home health industry, advancements in nursing practice and job functions as requested.
15. Assists in the coordination of care with the infusion pharmacy and other outside entities as needed.
16. Facilitates the reporting of significant findings to the physician
17. Reviews lab results and reports abnormalities, as appropriate, to the physician.
18. Conforms with all agency policies and procedures
19. Assists in performing nursing competency assessments as requested
20. Participates in QAPI program as requested
21. Makes patient home visits as needed
22. Participate in surveys conducted by authorized inspection agencies.
23. Participate in the pharmacy’s Performance Improvement program as requested by the Performance Improvement Coordinator.
24. Participate in pharmacy committees when requested.
25. Participate in in-service education programs provided by the pharmacy.
26. Report any misconduct, suspicious or unethical activities to the Compliance Officer.
27. Perform other duties as assigned by supervisor.
Minimum Qualifications:
- Effective interpersonal, time management and organizational skills.
- Office experience preferred.
- Computer skills that include word processing, and efficient use of the internet and e-mail.
- Must possess excellent oral and written communication skills, with the ability to express technical issues in “layman” terms.
Education and/or Experience:
- Current California RN license in good standing
- Minimum of 1 year of clinical experience
- Current CPR certification
- Valid driver’s license
- Current auto liability insurance
- Must be able to communicate effectively in both verbal and written form
- Must have prior home health experience
- Infusion therapy experience is preferred
- Bilingual is preferred
- Prior supervisory experience in preferred
- Prior experience working in a home health office is preferred.
