Location: Tacoma, WA
Duration: Permanente (Full-Time)
Compensation: $150,000 per year + bonus potential

Job Overview:
Be a part of our mission to revolutionize care for individuals with Serious Mental Illness (SMI). As a Health Guide, you will lead and manage clinical assessments and provide crucial care coordination. You’ll also work directly with patients and collaborate with community resources to ensure a comprehensive support system.
Qualifications:
Master’s degree or higher with 3+ years in Primary Care or Value-Based Care
Active NP or PA-C license in WA
Coding experience preferred but not required
