Functional Medicine Provider
We are seeking a full-time (32-34 hours per week, hybrid) Functional Medicine Provider (Nurse Practitioner, Physician Assistant) to join our passionate team. The ideal candidate will have at least 1 year of clinical experience in Functional Medicine as well as certified in or actively pursuing certification in Functional Medicine. We are looking for someone who deeply believes in the power of understanding root causes and is passionate about providing individualized care.

Whether they’ve been diagnosed with conditions like thyroid dysfunction, GI issues, mold toxicity and more or they haven’t found answers yet, our clients value a holistic, integrative approach. They seek a partner who will work alongside them to identify root causes and create comprehensive solutions tailored to their unique health needs.
Role Outcomes:
- Empower Clients: Guide patients towards understanding their health, providing education, support, and encouragement throughout their healing journey.
- Personalized Care: Deliver individualized treatment plans that consider every aspect of a patient’s life—nutrition, hormones, exercise, and stress.
- High-Quality Management: Conduct follow-up appointments to ensure patients receive the continued care and support needed to achieve their goals.
- Collaborative Engagement: Work closely with our health coaches and the rest of the Trivida team to deliver seamless, compassionate care.
Roles and Responsibilities:
1: Patient Care & Education:
- Conduct in-depth lab reviews and follow-up appointments, focusing on addressing the root causes of health issues.
- Educate patients about lab results, treatment plans, and lifestyle adjustments.
- Design personalized supplement and treatment protocols.
2: Clinical Administration:
- Complete SOAP notes and manage patient records.
- Order labs, supplements, and manage referrals.
- Utilize advanced laboratory testing and nutritional assessments for comprehensive insights into patient health.
3: Communication & Team Collaboration:
- Respond to client messages proactively and compassionately, offering guidance when needed.
- Collaborate with health coaches to maintain cohesive patient care.
- Maintain accurate patient data using EHR systems and other technology platforms.
4: Patient Outcomes:
- 80% or better outcome for clients
- Monitor patient progress, track outcomes, and gather testimonials to support the growth of the practice.
- Stay informed about the latest in Functional Medicine, always seeking ways to improve the quality of care.

Experience and Requirements:
Education and Certification:
- Licensed Nurse Practitioner (NP) or Physician Assistant (PA).
- At least 1 year of clinical experience in Functional Medicine.
- Working towards a certification in Functional or Integrative Medicine is strongly preferred.
Skills and Attributes:
- Strong knowledge of Functional Medicine, including Anti-Aging and Integrative approaches.
- Effective communicator who can work with people from all walks of life.
- Ability to work collaboratively in a fast-paced, dynamic environment.

Compensation:
- Competitive salary based on experience ($90,000 – $110,000 annually)
- Opportunities for bonuses based on clinic performance

Availability:
- This is a full-time position with flexible hours, though availability for patient appointments and team meetings is required during regular working hours.
- Clinic is M-F, 9-5
- This role may involve some remote work, with the requirement for in-person care when needed.
