Shift: Day Shift (Hours: 8a-4:30) No Weekends or Holiday
Salary: $87,000.00 to $120,000.00 /Year + Health Benefit
Job Responsibility:
- This position will manage 10 Case Managers and 8 Social Workers.
- Manage the daily operations of the Care Management department staff and to provide effective and efficient discharge planning services to patients in accordance with policies, procedures, regulatory requirements.
Skills & Qualifications:
- Delaware RN License or compact state RN License
- Bachelor’s degree required.
- MSN or MBA preferred.
- Five years' experience in Case Management in an Acute Care hospital setting is preferred.
- Three years supervisory/management experience preferred.
