Rudish Health is currently seeking qualified candidates for an Interim Case Management Director for a client in Los Angeles, CA. The Interim Case Management/Utilization Review Director will utilize advanced nursing skills and knowledge of resource management and fiscal responsibility to coordinate the clinical care for a designated patient population across the continuum of care. The responsibilities include, but are not limited to, clinical effectiveness, discharge planning, care coordination, clinical resource management, and core measure documentation. This position interacts with Medical, Nursing, and Ancillary staff to facilitate quality-based, cost-effective patient outcomes and ensure care is provided in the most appropriate setting.

- Has knowledge of best practices and how their own area integrates with others.
- Demonstrates awareness of the industry, including regulatory, evolving customer demands, and the factors that differentiate the organization in the market.
- Assesses plans, implements, documents, coordinates, monitors, evaluates, and updates the plan of care by collaborating with all members of the health care team to provide evidence-based care.
- Establishes patient care management plans, interventions, treatment goals, and self-management goals.
- Assists patients with self-management through education, visits, and telephonic engagement; encourages and supports patient adherence to their care plans.
Qualifications:
- Hospital Case Management experience required
- Minimum of three years of clinical experience required
- Previous experience leading a case management team
EDUCATION
Bachelor’s degree required.
