ABOUT THE POSITION
OAM is seeking a Physical Therapist to join our Rehab team. This role includes evaluation, treatment and managing patients' care. In accordance with state laws, PTs carry out routine functions which include documenting the progress of treatment and modifying specific treatments in accordance with patient status and within the scope of treatment plans.

ESSENTIAL DUTIES & RESPONSIBILITIES
- Perform and document an initial exam including review their medical history and referrals from doctors or surgeons to identify problems, and determine a diagnosis before intervention.
- Provides direct patient care and develops an individualized treatment plan to maintain, improve or restore physical function, alleviate pain, and prevent physical dysfunction in patients.
- Maintain patient records by keeping track of goals and progress while adapting treatment plans in accordance with facility policies and procedures.
- Supervises Physical Therapy assistants to achieve patients' functional goals.
- Participate in team meetings and other clinical and staff meetings.
- Develop and enhance clinical expertise. Maintain awareness of current research, literature, and professional consensus regarding treatment strategies.
- Demonstrate adherence to state practice acts and codes of ethical professional conduct in all spheres of clinical practice.
- Discharge patients appropriately and follow up with patients as needed.
- Provide EMR documentation in a timely manner to support treatment and treatment goals.
- Perform related work and other duties as required and/or assigned.
EXPERIENCE & EDUCATION
- Physical Therapy degree from an accredited program
- Licensed Physical/ Occupational Therapist in State of Michigan
- CPR Certification preferred
- Must maintain CEU requirements of licensure
