Provides educational and clinical services in a clinical setting on disease or disorder self-management following clinical standards of care. Maintains up-to-date knowledge on disease or disorder related issues in order to serve as a resource to the patient community and participate in events designed to increase health awareness. Performs patient care activities for patients of diverse ages in a clinical setting. Advocates and utilizes the nursing process in the care of all age populations along the health continuum.
Minimum Education
- Graduate of an accredited school of nursing, BSN preferred or required as per specialty credentialing requirements.
Minimum Work Experience
- 2 year clinical experience in chronic disease care in specialty area being sought.
- Experience in educating a diverse population on healthcare issues is highly desirable.
Required Licenses/Certifications
- Licensed in the State of Vermont.
- BLS Certification through American Heart Association.
- Appropriate credentialing per specific discipline or to be acquired within 1 year of obtaining the position.

- Basic Microsoft Windows desktop application and navigation skills.
- Excellent verbal and written communication skills.
- Demonstrated organizational and planning skills.
- Ability to effectively communicate and relate information to individuals of varying ages from diverse socio-economic backgrounds.
Salary Range: $64,650.00 - $103,230.00
