Key Responsibilities
- Conduct Comprehensive Assessments: Analyze medical and psychosocial information, perform home visits, and evaluate client needs to identify appropriate services.
- Develop Personalized Care Plans: Assist clients in accessing home and community-based services that empower them to maintain independence in the least restrictive environment.
- Coordinate and Connect: Refer clients to resources, collaborate with healthcare professionals, and ensure a seamless support system through effective coordination of services.
- Champion Client Autonomy: Advocate for client rights, promote self-determination, and ensure equitable access to essential services.
- Document for Impact: Maintain accurate records to track progress, inform service decisions, and contribute to the continuous improvement of the AERS program.
Qualifications
Are you ready to make a real difference? We are seeking a candidate who:
- Holds a valid State of Maryland RN license.
- Has a minimum of 2 years of experience in medical-surgical, geriatric, and/or pediatric case management.
- Possesses strong assessment skills and a talent for tailoring support to meet individual client needs.
- Thrives in independent, home-based settings and approaches challenges with a positive, can-do attitude.
- Communicates with clarity and compassion, effortlessly building trust and rapport with clients.
- Is tech-savvy and comfortable navigating electronic medical records.
- Advocates for social justice and is dedicated to empowering vulnerable individuals.
- Can pass a thorough background investigation.
- Has a reliable laptop with hotspot access and dependable transportation.
- Holds a valid Maryland driver’s license (noncommercial, Class C).
- Is willing to travel to client homes throughout Baltimore City, Maryland.
- Is available for full-time work Monday to Friday, 8 AM to 5 PM (with potential weekend availability if candidate wants.)
