Are you passionate about providing holistic, compassionate care to individuals and families? At Third Street, we're dedicated to delivering high-quality healthcare to our community, and we're looking for an enthusiastic Family Medicine Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing team!
What You'll Do:
- Provide comprehensive, patient-centered care to individuals and families across the lifespan.
- Perform assessments, diagnose conditions, and develop personalized treatment plans.
- Collaborate with our multidisciplinary team to ensure integrated care for all patients.
- Educate patients and families on health maintenance, disease prevention, and healthy lifestyle choices.
- Participate in community outreach initiatives and health education programs.
- Maintain accurate and timely patient records in compliance with healthcare regulations.
Who You Are:
- APRN or PA State of Ohio License with a focus in Family Medicine.
- A strong communicator who builds rapport easily with patients and their families.
- Committed to health equity and culturally competent care.
- Energetic, adaptable, and eager to work in a collaborative environment.
- Experience in a community health setting is a plus but not required.
