Medix is looking to hire a Social Worker for the CalAIM program to join a growing healthcare management organization in Sacramento, CA.
Responsibilities:
- Meet with members to provide therapeutic supportive services addressing their mental health, functional, and substance use barriers so they can obtain and maintain housing.
- Serve as the primary point of contact for enrolled members.
- Provide crisis intervention services that enhance the members' abilities to independently solve problems, use effective coping skills, and manage self-care.
- Drive within the assigned territory to meet with members, when needed.
- Submit post visit documentation in a timely manner.
- Participate in ongoing team meetings.
- Additional duties, as needed.
Requirements / Experience:
- Bachelor’s Degree in Sociology, Social Work or Psychology
- 2 years of experience in homeless services, veteran services, mental health, substance use, co-occurring disorders, or with vulnerable populations providing intensive case management service.
- A valid driver’s license, auto insurance and a reliable vehicle to commute
Preferred Qualifications:
- Training in Motivational Interviewing, Trauma Informed Care, and other related training are preferred.
Schedule:
- Monday - Friday 8:00 -5:00 pm
- Hybrid model
Pay range:
$25-30/hr BOE
