Description
Starting compensation: $95,000.00 per year.
- Ensures the implementation, and enforcement of all clinical policies and procedures, including resident rights and keeps the Clinical Director informed.
- Actively participates in the hospice quality improvement program, including identifying opportunities for improvement, problem solving and implementing recommendations
- Provides hands on patient care when needed, provides education and teaching, medications, services, and prescribed treatments including those requiring professional nursing skills.
- Completes introductory and annual reviews for direct reports. Coaches, trains and administers corrective action to employees as required in collaboration with the Clinical Director.
- Ensures the provision of RN & CHHA/CNA orientation, education and in-services; to include personnel records maintain compliance with mandatory educational programs and health-related requirements.
- Maintains liaison relationships and communication with facility staff and services and with residents and their families. Represents the Home for Hospice in outside venues as requested
- Coordinates services to maintain continuity of safe, effective patient care and actively participates in interdisciplinary team conferences.
- Consults with physicians, hospice team members, patients and/or family members while establishing a coordinated hospice referral.
- Communicates effectively with patients, family, staff and physicians in order to ensure delivery of quality care and services.
- Ensures that medications are being dispensed, stored, and disposed of per regulations.
- Responsible to learn and utilize technology made available by the agency to increase efficiency, communication and performance of duties.
- Ensure all chart audits are complete in a timely manner per regulations.
- Maintains ongoing review of patient charts to ensure that documentation is accurate and concise and that all charts are closed at discharge.
Ensures compliance with Medicare, NJ Department of Health and CHAP regulations. - Facilitates cohesiveness and open communication within the organization.
- Serves as a customer service advocate, facilitating rapid response to all referrals and patient needs.
- Communicates with Intake Coordinator to prioritize admissions on a daily basis for the Home for Hospice. Ensures all patients admitted to the Residence meet the appropriate criteria for Hospice services per Medicare guidelines.
- Review and ensures accuracy of new admission chart generation and computer data; reviews and gives 2nd level approval to all hospice 485's.
- Reviews all documentation in patient charts to ensure it meets the standards of the Conditions of Participation, CHAP and NJDHSS.
- Ensure that documentation is accurate and concise and that all charts are closed at discharge.
- Responsible for coordination of RN&CHHA/CAN schedules; maximizes efficiency of patient care through monitoring of staff assignments based on acuity.
- Holds in strictest confidence all patient information and discloses information and data only to persons authorized by patient/family/representative or Hospice personnel.
- Utilizes policy and procedure manuals and other available current reference materials to ensure compliance and quality of care.
- Conducts and participates in team meetings.
- Participates in committees as assigned: PAC, Administration, Policy Review, QAPI
- Ensures that all Emergency Preparedness/Disaster/Fire Drills are being conducted in collaboration with the Facilities Manager.
- Facilitates cohesiveness and open communication within the organization.
- Demonstrates appropriate infection control measures when dealing with inpatient residents.
- Adheres to employment policies as described in the employee handbook; maintains and respects confidentiality; adheres to all HIPAA regulations.
- Participates in all required in service education programs and meetings.
- Demonstrates compliance with the dress code policy by appearing well groomed, wearing ID badge while on duty and maintaining a professional appearance.
- Performs other duties as assigned.
Position Requirements
- Knowledge of hospice conditions of participation, inpatient hospice regulations, and related policies & procedures.
- Clinical knowledge to support the dying process.
- Strong leadership ability.
- Ability to serve as a resource to patients and families during times of high stress and volatility
- Excellent interpersonal and communication skills, including providing for patients and families, members of the clinical team and employees.
- Ability to comprehend the spoken and written English language, to communicate with clarity both verbally and in writing.
- Well organized to manage patient/family needs, provide support to the clinical and oversight of the inpatient Hospice operation.
- Self-starter with the ability to work both independently with sound decision-making, and as a part of an interdisciplinary team.
- Strong assessment and documentation skills.
- Proficiency in operating a computer; including a working knowledge of Microsoft applications (Word, Excel, and Outlook) and an electronic medical record.
Education and Expterience:
Required Education: Associate degree in Nursing. Licensed to practice as a Registered Nurse in the State of New Jersey; CPR/AED Certification.
Preferred Education: Bachelor of Science in Nursing. CHPN Certification.
Required Experience: Minimum one-year of clinical experience in an acute-care setting.
Preferred Experience: Prior hospice and hospice supervisory experience preferred.
