Worksite Setting: FQHC
Scope of Work: Primary Care treating patients 18+. The first patient of the day is scheduled at 8AM, and the last patient of the morning is scheduled at 11:15AM. The first patient of the afternoon is scheduled for 1PM, and the last patient of the day is scheduled at 4:00PM. New patient/hospital follow up slots are 30-minutes appointments, and there are 4 of them. The remaining appointments are 15-minutes established patient slots.
Duties/ Procedures: D iagnose, treat, educate, follow up, and care for patients in accordance with the American Board of Family Practice standards/guidelines. Patient chart completion within 72 hours of patient visit. Completion of normal Telephone tasks within 24hrs of receipt. Completion of urgent telephone task on the same day of receipt. Reviewing and addressing all Lab/Imaging results within 72 of receipt with urgent labs within 24 hours and critical labs immediately on receipt. Reviewing and addressing all Documents/Referrals within 72 hours of receipt. Urgent documents/referrals within 24 hours. Reviewing and addressing all medication requests within 72 hours of receipt and urgent requests as per medication services within 24 hours. Adhering to the provider rotation schedule as set forth by the CMO which is subject to change based on provider call out/illness and patient needs. The provider needs to be able to perform PAP smears, clinical breast exam, digital rectal exams where medically appropriate with proper chaperones present. The provider needs to be able to perform removal of sutures, removal of staples, and have general wound care/surgical site care knowledge. All other procedures such are joint injections, suturing, I&Ds, etc., are welcomed but not required.
